A gut model built from patient cells fingers an overlooked culprit

A Colon Chip built from patients' own cells found supposedly structural fibroblasts actively driving gut inflammation.

Researchers have built an organ-on-chip model of inflammatory bowel disease, a device they call the Colon Chip, using cells donated by patients with Crohn's disease and ulcerative colitis, the two main forms of IBD. The work was published in Nature Biomedical Engineering, dated July 20, 2026, with Harvard bioengineer Alican Ozkan as first author. The chip brings together matched patient-derived epithelial cells, stromal fibroblasts, circulating immune cells, and peristalsis-like mechanical forces on a single device.

The central finding turns a common assumption around: fibroblasts, usually treated as passive structural support, appeared to actively drive inflammation and weaken the gut's protective barrier. Exposing healthy intestinal cells to fibroblasts from IBD patients was enough to make the healthy tissue behave like diseased tissue, with a leakier barrier and stronger inflammatory responses. Mechanical stretching that mimics normal bowel movement intensified inflammatory and fibrotic responses in IBD chips, pregnancy-related hormones raised inflammation and collagen deposition in female patients' cells, and exposure to the carcinogen ENU produced far stronger cancer-associated changes in diseased chips.

IBD affects millions of people worldwide, and its mechanisms remain poorly understood. A model that pins down a driver and lets researchers test hypotheses in human tissue could shorten the path to treatments and make target discovery cheaper than trial-and-error in patients.

Watch whether the fibroblast lead points to a drug target. The caveats are real: the 'first model' and 'to my knowledge' claims come from the study's first author, the results are from a lab chip rather than living patients, and the work builds on a 2022 PLOS Biology study that already showed colon fibroblasts adopt inflammatory states during chronic inflammation.
