An Ebola outbreak has killed 1,707 people, and the vaccines that work do not work on it

Congo's Bundibugyo outbreak has 3,802 confirmed cases since May. Africa CDC says contact tracing is not working. Moderna began a Phase 1 trial on August 4.

An Ebola outbreak declared in eastern Congo on May 15 had produced 3,802 confirmed cases and 1,707 deaths as of August 4. Nearly 90% are in Ituri province, with cases confirmed in five other provinces including Kisangani. More than 100 health workers have been infected.

The reason this is worse than the case count suggests is the pathogen. This is Bundibugyo ebolavirus. The licensed Ebola vaccines protect against Zaire ebolavirus, a different species — so for this outbreak there is no approved vaccine and no approved treatment. The tool that ended previous epidemics does not fit this one.

Containment is failing on its own terms. Africa CDC's director-general has said plainly that contact tracing is not working: 60 to 70% of new cases are arising from community spread rather than among traced contacts, despite more than 17,000 contacts under monitoring with nearly 80% seen daily and over 2,000 samples tested a day. Health worker strikes over unpaid wages have compounded it, and patient zero has still not been identified.

Four trials are running — two for post-exposure prophylaxis and two for vaccines, in Ituri, the UK and Canada. Separately, Moderna announced on August 4 that it had begun dosing an approximately 80-person Phase 1 trial of mRNA-1469 against this strain, with up to $50 million from CEPI and a commitment to a minimum of 500,000 doses for low- and middle-income countries at access pricing.

That is the honest shape of it. On August 1 this site carded Uganda declaring itself free of Ebola. Four days later, the strain nobody built a vaccine for is outrunning the response, and the first human dose against it went into an arm the same week.
