Arthritis Drug Shows Hair Regrowth for Severe Alopecia in Clinical Trials

Upadacitinib, a drug developed for arthritis, demonstrated hair regrowth in two Phase 3 clinical trials for severe alopecia areata. The studies enrolled 1,399 participants aged 12–64 years, including 118 adolescents, all with ≥50% scalp hair loss (SALT score ≥50). After 24 weeks, 55% of patients on 30 mg daily dosing achieved at least 80% scalp hair coverage (SALT score ≤20), while 45.2% on 15 mg dosing reached this threshold. Full scalp hair regrowth occurred in 13–22.5% of participants on upadacitinib versus 1.5–3.4% on placebo. The drug was administered orally at 15 mg or 30 mg doses, with trials published in JAMA Dermatology (2026). This non-surgical treatment offers a potential pathway to address severe hair loss, reducing mental health burdens for patients who previously had limited options.

The mechanism involves the drug’s established anti-inflammatory action repurposed for autoimmune hair loss. Its efficacy in this application is significant given the trial’s large scale and focus on severe cases where conventional treatments often fail.
